§ 5.02.07. Issuance of licenses.

The service-safety director shall issue licenses to haulers in accordance with the terms of this ordinance and on behalf of the city, as follows: Applications for license under the provisions of this ordinance shall be filed with the service-safety director on such forms as the director shall prescribe. Such applications shall contain:

(A)

A statement of the name, address, and telephone number of the applicant;

(B)

A list of the corporate relations and/or of partners stockholders who own ten percent or more of the applicant corporation;

(C)

A description of the type and condition of the equipment to be used for collection, transportation, and disposition;

(D)

A statement that the method of disposition to be used is in agreement with national, state, and local health standards; and

(E)

Such other facts as may be required by the service-safety director.
