§ 11.04.04. Licensing procedure.

Every person desiring a license for vending, peddling or soliciting shall file an application with the code enforcement office upon a form provided by said office .

The application shall contain the following information:

(A)

The name, mailing address, permanent address, telephone number and social security number of the applicant;

(B)

Federal and state employer's identification number, and sales tax identification number, if applicable;

(C)

A description of the product or service to be vended, peddled or solicited;

(D)

The names, addresses and telephone numbers of at least three business references to prove applicant's credibility;

(E)

The supplier of the applicant's merchandise, or by whom the services solicited shall be rendered, unless the said applicant shall supply the goods or services;

(F)

Certificate of insurance for category "A" and "B" vendors. Liability insurance with the City of Athens as the certificate holder will be required with a minimum of $100,000.00 for property and $300,000.00 for bodily injury/wrongful death insurance.

(G)

Any other information the code enforcement office shall deem necessary.
